CONCERNS :

ACNE VULGARIS

Acne vulgaris (or simply acne) is a long-term skin disease that occurs when hair follicles become clogged with dead skin cells and oil from the skin. Acne is characterized by areas of blackheads, whiteheads, pimples, and greasy skin, and may result in scarring. The resulting appearance can lead to anxiety, reduced self-esteem

           Pathophysiology of Acne:

Comedogenesis and acnegenesis are actually discrete processes, but they are usually associated with one another, with the latter often succeeding the former.

Inflammation of the follicular epithilium, which loosens hyperkinetic material within the follicle creating pustules ,characterizes acnegenesis.Comedogenesis is best described as a noninflammatory follicular reaction manifested by a dense compact hyperkeratosis of the follicle, and usually precedes Acnegenesis.Because of the etiology of the such lesions varies from person to person and within individuals also,it is difficult to categorically identify or isolate a basic cause of acne.

Genetics is thought to be the cause in 80% of cases. The role of diet and cigarette smoking is unclear while neither cleanliness nor sunlight appear to be involved. Acne primarily affects skin with a greater number of oil glands, including the face, upper part of the chest, and back.During puberty, in both sexes, acne is often brought on by an increase in androgens such as testosterone. Excessive growth of the bacteria Propionibacterium acnes, which is normally present on the skin, is often involved.

1.Chemical Peeling

A chemical peel is a technique used to improve the appearance of the skin on the face, neck or hands. A chemical solution is applied to the skin that causes it to exfoliate and eventually peel off. The new, regenerated skin is usually smoother and less wrinkled than the old skin Superficial or lunchtime peel: Alpha-hydroxy acid or another mild acid is used to penetrate only the outer layer of skin to gently exfoliate it.

The treatment is used to improve the appearance of mild skin discoloration and rough skin as well as to refresh the face, neck, chest or hands. Medium peel: Glycolic or trichloroacetic acid is applied to penetrate the out and middle layers of skin to remove damaged skin cells. The treatment is used to improve age spots, fine lines and wrinkles, freckles and moderate skin discoloration. It also can be used to smooth rough skin and treat some precancerous skin growths, i.e. actinic keratosis. Deep peel: Tricholoracetic acid or phenol is applied to deeply penetrate the middle layer of skin to remove damaged skin cells.

The treatment removes moderate lines, age spots, freckles and shallow scars. Patients will see a dramatic improvement in skin appearance. The procedure is used on the face and only can be performed once. Indication- Acne scars Aging skin Crow's feet Scars Sun damaged skin Sagging skin Wrinkles

2.Laser Treatment

Laser resurfacing is a treatment to reduce facial wrinkles and skin irregularities, such as blemishes or acne scars. The technique directs short, concentrated pulsating beams of light at irregular skin, precisely removing skin layer by layer. If aging, acne, or too much time in the sun has left your face with blotches, scars, wrinkles, or lines, laser skin resurfacing may help your skin look younger and healthier.

3 Medi Facials

Medi-facials are skin treatments formulated to improve general skin health as well as to address specific skin conditions. Astique has selected high-quality science-based products to revitalise, heal and refresh your skin. Clinically-proven brands which use only natural, highly-functional and virtually irritation-free ingredients leave you looking younger and more radiant.

It's simply an enhanced facial geared towards achieving specific results designed to repair, restore, and rejuvenate the skin. Your skin will have visible improvements in tone, texture, complexion, and most importantly improved skin health and function.

Following Medifacials available:

    .Hydra-facials

  • Oxygen facials that uses oxygen in between facials.
  • Facials using electroporation devices: These devices      are used for infusing vitamin C, glutathione, and other essential      anti-oxidants directly into the skin.
  • Vampire PRP or stem cell facials: These are done with      patient's own blood and are an obvious solution for arresting the clock of      ageing and for a good youthful skin for a long time.

Pigmentation

Pigmentation means coloring. Skin pigmentation disorders affect the color of your skin. Your skin gets its color from a pigment called melanin. Special cells in the skin make melanin. When these cells become damaged or unhealthy, it affects melanin production. Some pigmentation disorders affect just patches of skin. Others affect your entire body.

If your body makes too much melanin, your skin gets darker. Pregnancy, Addison's disease, and sun exposure all can make your skin darker. If your body makes too little melanin, your skin gets lighter. Vitiligo is a condition that causes patches of light skin. Albinism is a genetic condition affecting a person's skin. Infections, blisters and burns can also cause lighter skin.

Pigment disorders can be widespread and affect many areas of skin, or they can be localized and affect only certain areas of the skin.

Scars

Scars are areas of fibrous tissue (fibrosis) that replace normal skin after injury. A scar results from the biological process of wound repair in the skin and other tissues of the body. Thus, scarring is a natural part of the healing process

Scars always occur when tissues have been significantly damaged and repaired.
Scars produce changes that disturb the physical architecture of normal skin or other tissue.
Scars can occur after physical trauma or as part of a disease process.

Warts

Warts are skin growths that are caused by the human papillomavirus (HPV). There are more than 60 kinds of HPV, some of which tend to cause warts on the skin. HPV stimulates quick growth of cells on the skin's outer layer. In most cases, common warts appear on the fingers, near the fingernails, or on the hands. Certain types of HPV can also cause warts to appear in the genital area.

Plantar warts: This type of wart usually appears as flesh-colored or light brown lumps that are flecked with small clotted blood vessels that appear as tiny black dots. Plantar warts appear on the soles of the feet.

Genital warts: Genital warts can appear in the pubic area, on the genitals, in or around the anus, and/or in the vagina. They look like small flesh-colored, pink, or red growths. The warts may look similar to the small parts of a cauliflower or they may be very tiny and difficult to see. They often appear in clusters of three or four, and may grow and spread rapidly. They usually are not painful, although they may cause mild pain, bleeding, and itching. HPV infection is the most common sexually transmitted disease in North America and certain forms of the virus can cause cervical cancer.

Flat warts: This type of wart is more common in teens and children than in adults. Flat warts are smoother, flatter, and smaller than other warts and they generally occur on the face. Flat warts also can appear on the legs, especially among females.

Moles

Moles are a common type of growth on the skin. They often appear as small, dark brown spots and are caused by clusters of pigmented cells. Moles generally appear during childhood and adolescence. Most people have 10 to 45 moles, almost all of which appear before age 40. Some moles may fade or disappear as you age.

Most moles are harmless. Rarely, they become cancerous. Monitoring moles and other pigmented patches is an important step in detecting skin cancer, especially malignant melanoma. The medical term for moles is nevi.

Wrinkles

A wrinkle, also known as a rhytide, is a fold, ridge or crease in the skin. Skin wrinkles typically appear as a result of aging processes such as glycation, habitual sleeping positions, loss of body mass, or temporarily, as the result of prolonged immersion in water. Age wrinkling in the skin is promoted by habitual facial expressions, aging, sun damage, smoking, poor hydration, and various other factors.

Hair Fall

Hair loss is a very common condition and affects most people at some time in their lives. androgenetic hair loss is seen in both men and women but is worse in men.
Thyroid disease, anemia, protein deficiency, and low vitamin levels may cause hair loss.
Alopecia areata is a relatively common cause of hair loss that usually resolves on its own.
Medications indicated for hair regrowth include minoxidil and finasteride, But it has major side effects in long term use
Prevention of hair loss include
good hair hygiene, regular shampooing, and good nutrition.
Medical health screening for hair loss may include blood tests such as complete blood count (CBC), iron level, vitamin B, and thyroid function tests (TFT).

Causes of Hair loss

1) Hormonal Changes- Hormonal changes and imbalances can cause temporary hair loss. This could be due to pregnancy, childbirth or the onset of menopause.

    a) Androgenetic hair loss is seen in both men and women but is more dramatic in men.

    b) Thyroid disease, anemia, protein deficiency, chemotherapy, and low vitamin levels may cause hair loss.

    c) Alopecia areata is a form of hair loss produced by the autoimmune destruction of hair follicles in localized areas of skin.

2) Stress- Both emotional and physical stress (such as a serious illness or recovery from surgery) have been associated with hair loss. It is possible that stress induces hormonal changes that are responsible for the hair loss, since hair loss is a known consequence of other hormonal changes due to pregnancy, thyroid disturbances, or even from taking oral contraceptives.

3) Family history (heredity)- The most common cause of hair loss is a hereditary condition called male-pattern baldness or female-pattern baldness. It usually occurs gradually and in predictable patterns — a receding hairline and bald spots in men and thinning hair in women. Heredity also affects the age at which you begin to lose hair, the rate of hair loss and the extent of baldness. Pattern baldness is most common in men and can begin as early as puberty. This type of hair loss may involve both hair thinning and miniaturization (hair becomes soft, fine and short).

4) Hair- pulling disorder. This condition, also called trichotillomania causes people to have an irresistible urge to pull out their hair, whether it's from the scalp, the eyebrows or other areas of the body.

5) Medications- Hair loss can be caused by drugs used for cancer, arthritis, depression, heart problems, high blood pressure and birth control. Intake of too much vitamin A may cause hair loss as well.

6)Other causes of hair loss.
-Radiation therapy to the head. The hair may not grow back the same as it was before.
-Certain hairstyles and treatments. Excessive hairstyling or hairstyles that pull your hair tight, such as pigtails      or cornrows, can cause traction alopecia. Hot oil hair treatments and permanents can cause inflammation of      hair follicles that leads to hair loss. If scarring occurs, hair loss could be permanent.

Most people normally shed 50 to 100 hairs a day. This usually doesn't cause noticeable thinning of scalp hair because new hair is growing in at the same time. Hair loss occurs when this cycle of hair growth and shedding is disrupted or when the hair follicle is destroyed and replaced with scar tissue.

Dandruff

Dandruff is the shedding of dead skin cells from the scalp.[1] As skin cells die, a small amount of flaking is normal; about 487,000 cells/cm2. Some people, however, experience an unusually large amount of flaking either chronically or as a result of certain triggers, up to 800,000 cells/cm2, which can also be accompanied by redness and irritation. Dandruff is a common scalp disorder affecting almost half of the population at the post-pubertal age and of any sex and ethnicity. It often causes itching. It has been well established that keratinocytes play a key role in the expression and generation of immunological reactions during dandruff formation. The severity of dandruff may fluctuate with season as it often worsens in winter.[2] Dandruff is rare before puberty, peaks in the teens and early twenties, and declines with age thereafter.[3] Most cases of dandruff can be easily treated with specialized shampoos. There is, however, no true cure.

Baldness

Hair loss, also known as alopecia or baldness, refers to a loss of hair from the head or body. Baldness can refer to general hair loss or male pattern hair loss. Hair loss and hypotrichosis have many causes including androgenetic alopecia, fungal infection, trauma (e.g., due to (trichotillomania), radiotherapy, chemotherapy, nutritional deficiencies (e.g., iron deficiency), and autoimmune diseases (e.g., alopecia areata). Hair loss severity occurs across a spectrum with extreme examples including alopecia totalis (total loss of hair on the head) and alopecia universalis (total loss of all hair on the head and body). Baldness is the partial or complete lack of hair growth, and part of the wider topic of "hair thinning". The degree and pattern of baldness varies, but its most common cause is androgenic alopecia, alopecia androgenetica, or alopecia seborrheica

Alopecia Areata

Alopecia areata (AA), also known as spot baldness and or 'Davey Kirts syndrome' is an autoimmune disease in which hair is lost from some or all areas of the body, usually from the scalp due to the body's failure to recognize its own body cells and destroys its own tissue as if it were an invader. Often it causes bald spots on the scalp, especially in the first stages. In 1–2% of cases, the condition can spread to the entire scalp (alopecia totalis) or to the entire epidermis (alopecia universalis). Conditions resembling AA, and having a similar cause, occur also in other species.

Commonly, alopecia areata involves hair loss in one or more round spots on the scalp.
Hair may also be lost more diffusely over the whole scalp, in which case the condition is called diffuse alopecia areata.
Alopecia areata monolocularis describes baldness in only one spot. It may occur anywhere on the head.
Alopecia areata multilocularis refers to multiple areas of hair loss.
Ophiasis refers to hair loss in the shape of a wave at the circumference of the head.
The disease may be limited only to the beard, in which case it is called alopecia areata barbae.
If the patient loses all the hair on the scalp, the disease is then called alopecia totalis.
If all body hair, including pubic hair, is lost, the diagnosis then becomes alopecia universalis.
Alopecia areata totalis and universalis are rare.

Obesity:

Obesity is a medical condition in which excess body fat has accumulated to the extent that it may have an adverse effect on health. It is defined by body mass index (BMI) and further evaluated in terms of fat distribution via the waist–hip ratio and total cardiovascular risk factors. BMI is closely related to both percentage body fat and total body fat.

(BMI), a measurement obtained by dividing a person's weight by the square of the person's height, is over 30 kg/m2, with the range 25–30 kg/m2 defined as overweight

Obesity is most commonly caused by a combination of excessive food intake, lack of physical activity, and genetic susceptibility. A few cases are caused primarily by genes, endocrine disorders, medications, or mental disorder. The view that obese people eat little yet gain weight due to a slow metabolism is not generally supported. On average, obese people have a greater energy expenditure than their normal counterparts due to the energy required to maintain an increased body mass.

Obesity is one of the leading preventable causes of death worldwide. A number of reviews have found that mortality risk is lowest at a BMI of 20–25 kg/m in non-smokers and at 24–27 kg/m2 in current smokers, with risk increasing along with changes in either direction. This appears to apply in at least four continents. In contrast, a 2013 review found that grade 1 obesity (BMI 30-35) was not associated with higher mortality than normal weight, and that overweight (BMI 25-30) was associated with "lower" mortality than was normal weight (BMI 18.5-25).Other evidence suggests that the association of BMI and waist circumference with mortality is U- or J-shaped, while the association between waist-to-hip ratio and waist-to-height ratio with mortality is more positive. In Asians the risk of negative health effects begins to increase between 22–25 kg/m2. A BMI above 32 kg/m2 has been associated with a doubled mortality rate among women over a 16-year period. In the United States, obesity is estimated to cause 111,909 to 365,000 deaths per year, while 1 million (7.7%) of deaths in Europe are attributed to excess weight. On average, obesity reduces life expectancy by six to seven years, a BMI of 30–35 kg/m2 reduces life expectancy by two to four years, while severe obesity (BMI > 40 kg/m2) reduces life expectancy by ten years.

Obesity usually results from a combination of causes and contributing factors like-

· Genetics

· Lifestyle

· Inactivity

· Unhealthy diet

· Certain illness

· Medications

· Social and economic issues

· Age

· Pregnancy

· Lack of sleep

Obesity more likely to have conditions including

· Heart disease and stroke

· High blood pressure

· Diabetes

· Some cancers

· Gallbladder disease and gallstones

· Osteoarthritis

· Gout

· Breathing problems, such as sleep apnea (when a person stops breathing for short episodes during sleep) and asthma

Unwanted Hair Growth/ Hirsutism

If you're a woman and you have a lot of hair growing in places where it normally does just for men, like your upper lip, chin, chest, stomach, or back, that's a condition called hirsutism.

The hair is often dark and coarse, instead of the light, fine "peach fuzz" that covers most of the body. CausesIt's often caused by genes, hormones, or medication.Genes. Sometimes, hirsutism runs in families. If your mother or sisters have it, you're more likely to get it. It's also more common in people from the Middle East, South Asia, and the Mediterranean.

Hormones. Many times, the condition is linked to high levels of male hormones (called androgens). It's normal for women's bodies to make these, and low levels don't cause excess hair growth. But when these amounts are too high, they can cause hirsutism and other things, like acne, a deep voice, and small breasts.
High levels of male hormones and hirsutism are common in women who have:
Polycystic ovary syndrome , which causes small cysts, or fluid-filled sacs, to form on your ovaries.
Cushing's syndrome, which you get when you have high levels of the stress hormone cortisol for long periods of time.
Tumors in your adrenal glands (which make hormones like cortisol) or your ovaries.

Electrocautery

Electrocautery, also known as thermal cautery, refers to a process in which a direct or alternating current is passed through a resistant metal wire electrode, generating heat. The heated electrode is then applied to living tissue to achieve hemostasis or varying degrees of tissue destruction. Electrocautery can be used in various minor surgical procedures in dermatology, ophthalmology, otolaryngology, plastic surgery, and urology.

Indications

Electrocautery is a safe and effective method of hemostasis during cutaneous surgery. It is also useful in the treatment of various small benign skin lesions, although only lesions that do not require histological review should be treated with electrocautery.

Electrocautery shares many indications with electrosurgery and is of particular importance in patients who have implanted electrical devices in whom external electromagnetic interference should be avoided. Furthermore, unlike electrosurgical instruments, electrocautery devices maintain function in a wet field.

Low temperatures can be used for superficial tissue destruction in the treatment of superficial and relatively avascular lesions, including the following:

· Seborrheic keratoses

· Acrochordons

· Molluscum

· Verrucae/ Warts

· Syringomas

· Small angiomas

· Moles

·

Dermabrasion

Dermabrasion is an exfoliating technique that uses a rotating instrument to remove the outer layers of skin, usually on the face. This treatment is popular with people who wish to improve the appearance of their skin.

Dermabrasion is a type of surgical skin planing, typically performed in a professional medical setting by a dermatologist or plastic surgeon trained specifically in this procedure. Dermabrasion has been practiced for many years (before the advent of lasers) and involves the controlled deeper abrasion (wearing away) of the upper to mid layers of the skin with any variety of strong abrasive devices including a wire brush, diamond wheel or fraise, sterilized sandpaper, salt crystals, or other mechanical means. Dermabrasion should not be confused with microdermabrasion which is a newer and non-surgical cosmetic procedure performed by non-physician personnel, nurses, estheticians, medical assistants,

· acne scars

· age spots

· fine wrinkles

· precancerous skin patches

· rhinophyma, or redness and thick skin on the nose

· scars from surgery or injury

· sun damage

· uneven skin tone

Laser Treatment

Laser resurfacing is a treatment to reduce facial wrinkles and skin irregularities, such as blemishes or acne scars. The technique directs short, concentrated pulsating beams of light at irregular skin, precisely removing skin layer by layer. If aging, acne, or too much time in the sun has left your face with blotches, scars, wrinkles, or lines, laser skin resurfacing may help your skin look younger and healthier.

Skin Tightening

Skin tightening is anti-aging treatment which is used for wrinkle reduction, fat reduction, loose skin and the alleviation of other signs of aging. Machines which are used for skin tightening are Thermage and Lasers .

Laser skin tightening is a minimally invasive, non-surgical process that uses an infrared light source (a laser) to tighten skin by heating the collagen under the skin's surface, causing the skin to contract (tighten). Facial skin tightening is noticeable immediately after the treatment, and there is no downtime, making this an increasingly popular procedure. Additional skin tightening occurs over the next few months, but optimal results usually require two or three treatments about a month apart.Laser skin tightening is an FDA approved method for the reduction of fine lines, wrinkles, and skin laxity. While laser skin tightening results may not be as dramatic as those of a face lift, patients enjoy moderate results with no downtime. An added benefit of laser skin tightening is that it is safe and effective for restoring a more firm, youthful appearance to skin all over the body.

Thermage is a safe, non-invasive, radiofrequency (RF) cosmetic procedure that's clinically proven to help smooth and improve skin for an overall younger looking appearance. The treatment delivers natural looking results with little to no downtime—on all skin types on and off the face, all in a .

Derma Roller / Microneedling

Micro-needling is a general term for a process that involves moving a special device over your skin that has a roller with many tiny (usually metal) needles embedded in it. There are different kinds of micro-needling devices with different product names. One is the manual version of micro-needling called a Dermaroller. There are also motorized devices such as the Dermapen or Dermastamp. For the most part, these micro-needling devices have reasonable science behind them indicating they may help in reducing the appearance of scars, but there's far less support for their ability to address wrinkles or for their ability to help anti-aging ingredients absorb better into skin. Here are the various options:

Dermapens look exactly like a pen, with a circular head studded with tiny needles. The head and needles are motorized. The motor-driven needles move in and out of the skin in a stamping motion, piercing it with thousands of tiny punctures.

Benefits of Micro-needling and Derma Rollers

There are three primary uses for micro-needling devices, but not all of them are beneficial. The first, which has some good research behind it, is to break down the thick collagen that causes some types of scarring.

You may have seen claims that these needling procedures can also reduce cellulite. However, whether or not micro-needling of any kind can work on cellulite is at best dubious. That's because cellulite involves fat deposits in connective tissue on the legs and buttocks, and needling skin cannot change any of this, not even a little. If you see results on your cellulite from needling, it's most likely due to the inflammation from the needles, not because cellulite was reduced. Once the inflammation subsides, the cellulite looks just like it did before needling. So the needling can help, but just a little and the results will be temporary. In terms of building collagen, the Dermapen and Dermastamp have emerging research about their benefits for wrinkles as an easy procedure that's far less expensive than other devices or treatments. In contrast, the Dermaroller has no such research, though again, theoretically there's logic to the concept.

Mesotherapy

Mesotherapy for hair regrowth involves the technique of injecting nutrient rich liquid in the mesoderm of the scalp – the layer under the epidermis which serves as a link between the layer of fats and the tissues of the skin. This is done with the help of a special mesotherapy gun that carries a sterile needle which injects the solution in the scalp. This treatment usually spans over a period of eight sessions, one every week for a duration of half an hour each. One must be patient and composed while taking this treatment as visible results are only observed after the fifth session.

he basis of the treatment substance that is injected during the therapy depends on the cause for the hair fall experienced. Since this substance generally contains vitamins, nutrients and other tissue friendly materials, this treatment is considered safe with usually no side effects or allergic reactions. The patients who undergo this treatment experience no or minimal pain and least discomfort making mesotherapy an easy solution for getting rid of the chronic problem of balding.

Another hair and scalp problem that can be treated by mesotherapy is dandruff. This alternative should however, only be used in case of severe dandruff problems.

PRP

PRP therapy for hair loss is a treatment that involves withdrawing a patient's own blood, processing it so that only the enriched cells (platelet-rich plasma) remain, and injecting it into the scalp. PRP contains essential proteins that stimulate natural hair growth. The treatment can also be combined with hair transplant surgery, microneedling, or medications

Advantages of PRP Therapy against hair loss are:

· Simple, non surgical procedure

· Multiple Injections: procedure lasting approximately 60 minutes

· Safe and reliable results

· Very quick recovery period

· Beautiful and above all extremely natural looking end results

Low Level Laser Therapy

Low level laser therapy is a safe form of light/heat treatment under investigation for a variety of health indications. It is being used to treat the genetic forms of hair loss common in men and women, androgenetic alopecia or pattern balding.

Low level laser therapy is also called red light therapy, cold laser, soft laser, bio-stimulation and photobiomodulation.

Laser therapy used for hair loss treatment depends on devices that emit a light that can penetrate the scalp. Excimer (308 nm), helium-neon (632.8 nm), and fractional erbium-glass (1550 nm) lasers have been used. Low level laser therapy (LLLT) uses devices with diodes that emit red light (wavelength 630-670 nanometers)

Low level laser therapy is intended for men and women with thinning hair or pattern baldness caused by a hereditary condition.

Physicians use a system known as the Norwood-Hamilton Classification (men) and the Ludwig-Savin Scale(women) to describe the degree of hair loss. Low level laser therapy is intended for promotion of hairgrowth in males who have Norwood Hamilton Classifications of IIa to V and in females who have Ludwig (Savin) I-4, II-1, II-2, or frontal patterns of hair loss and have Fitzpatrick skin phototypes I to IV.

Medication

Certain Vitamins & Minerals are prescribed to correct the nutritional deficiency , In Thyroid disorder or Anaemia proper medicines related to these diseases are needs to be taken.

Weight Loss Treatments

Low level laser light reduces the stability of adipocyte cell membranes, allowing cells to release their stores of fat without damaging the cell. Because the fat is released into the extracellular space, cardio based exercise is usually promoted after the procedure. Exercise increases lymph flow and is thought to also promote the use of the lipids as an energy source. This is also why adequate hydration is encouraged. The laser technologies are differentiated mainly by the wavelength of light used.

Weight Loss Treatments

Ultrasound

Focused thermal ultrasound techniques work by raising the tissue temperature above 56 °C, resulting in coagulative necrosis of adipocytes, with sparing of vessels and nerves. Passive heating of the skin may also induce collagen remodeling.

Weight Loss Treatments

Radiofrequency

Radiofrequency devices work by producing an alternating flow, which creates an electric field over the skin. The electric field shifts polarity millions of times per second, that causes a change in orientation of charged particles.
